You are entitled to enroll in the United Concordia group dental care plan. Carnegie does not contribute to the cost of this insurance. The premium is paid by payroll deduction in the last check of the month. Care is provided by a participating dentist selected by you.

To find a dentist that accepts the plan please go to the United Concordia website and select “Find a Dentist” On this page select “DHMO Concordia Plus” to find a dentist that accepts the plan that Carnegie offers. You may search under your address, zip code, or county where you live. The two main counties for this area are San Mateo County and Santa Clara County. Carnegie’s Zip Code is 94305.
