Part 1

Line 1 – Your name

Line 2 – Your Social Security number

Line 4 – Your address in your home country

Line 5 – US home address, NO P.O. BOX ADDRESSES

Line 6 – J1

Line 7a – Country issuing passport

Line 7b – Passport number

Line 8 – Last date of entry into the US as shown on your I-94 form

Line 9a – Your status is “Nonresident Alien

Line 9b – Date your current visa expires

Line 10 – check this box

Part II

Line 11a – write in “Research in Plant Biology

Line 11b – Total amount of compensation from date of form to end of year.

Line 12a – Specific treaty and Article (example:  US/Germany Article 20(1), etc.)

Line 12b – If all income is exempt write “All”.  If only a portion of the year will be exempt enter that amount (or if you are being employed mid-way through the year).

Line 12c – Write in your home country

Lines 13a – 13 c – DOES NOT APPLY TO CARNEGIE PERSONNEL

Line 14 – Write in:

  Exemption claimed under US Tax Treaty with home country as pertains to Professors, Teachers, and Researchers.  My position is a postdoctoral research associate.
