Volunteer Status
If you are going to be working in one of CarnegieÂ’s labs as a volunteer please complete the following. Everyone at our Departments (Plant Biology and Global Ecology) is required to complete the on-line Stanford Health & Safety Training. Please follow the instructions found on the following page:
ØVolunteer Information Form – Please complete as fully as possible. ØLetter to Director – This is your acknowledgement that you are an unpaid volunteer working in the lab with no expectation of payment or benefits, please read carefully and sign. ØHealth Insurance information ØEmergency Contacts – These forms are necessary in the event of an emergency so we have knowledge of your health insurance coverage and emergency contacts ØInvention Disclosure Agreement ØAccessibility of Data Agreement – Both are required for all scientific and technical staff |
